HIPPA Designation of Personal Representative

Under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), certain limitations are placed upon the disclosure by medical providers of a person’s medical information to third parties, including a spouse or other family members, without the person’s express consent. The HIPAA Designation of Personal Representative is a document in which a person designates individuals who may have full access to their medical information. This document does not give the designated individuals any decision making authority.
